Policies
Zenith Dental Arts is dedicated to delivering high-quality dental restorations with precision, consistency, and professionalism. To uphold our standards and ensure predictable outcomes, the following policies have been established. These policies define the responsibilities of both the prescribing clinician and the laboratory, and they govern all case submissions to Zenith Dental Arts.
1. Doctor’s Responsibility for Case Submission
The prescribing doctor is responsible for submitting all information required for the accurate fabrication of any dental restoration. Required information includes, but is not limited to:
- Accurate analog impressions or digital scans
- Correct stump shade (particularly for highly esthetic crowns and veneers)
- Accurate shade selection, including any custom, characterized, or special requests
- Clear written instructions regarding materials, design preferences, and additional requirements
Policy:
If essential information—such as shade or stump shade—is omitted, incomplete, or inaccurate, and the resulting restoration does not meet the expectations of the doctor or patient, Zenith Dental Arts will not be liable for a complimentary remake.
2. Shade Matching and Color Information
Shade accuracy depends entirely on the documentation provided by the prescribing doctor. High-quality shade photographs, stump shade documentation, and detailed shade mapping are strongly recommended. Any pictures taken chairside must be sent via the WhatsApp group along with the patient name attached on the email.
Policy:
If incomplete, inaccurate, or insufficient shade information is provided and the final shade does not meet expectations, the laboratory will not provide a complimentary remake. All remakes will be billed at standard rates.
3. Approval of Wax-Ups and Digital Designs
For cases requiring custom design, Zenith Dental Arts will provide a digital or physical wax-up for review. Any pictures taken chairside must be sent via WhatsApp along with the patient name attached on the email.
Policy:
Once a wax-up or design has been approved by the prescribing doctor, any modifications requested after final fabrication will be considered a new case. Full remake fees will apply.
4. Impression and Scan Accuracy
The accuracy and fit of any restoration are dependent upon the quality of the impression or digital scan provided.
Policy:
If impressions or scans are found to be distorted, incomplete, or clinically inadequate, the laboratory will notify the prescribing doctor and may request new records. If the doctor elects to proceed with inadequate records against laboratory recommendation, Zenith Dental Arts assumes no responsibility for fit-related complications. Any resulting remakes will be billed in full.
5. Turnaround Times and Rush Services
Zenith Dental Arts makes every effort to meet estimated turnaround times. However, cases submitted as “rush” requests inherently limit fabrication time.
Policy:
Rush cases may void remake guarantees. Doctors submitting rush requests assume full responsibility for any remake costs associated with insufficient processing time. A 30% rush fee may be applied at the discretion of Zenith Dental Arts.
6. Remakes Due to Laboratory Error
If a restoration error is determined to be the direct result of laboratory oversight—such as incorrect material selection or failure to follow clearly documented instructions—the restoration will be remade at no additional charge.
Zenith Dental Arts does not provide warranty coverage for prostheses damaged outside the patient’s mouth (e.g., accidental drops, excessive grinding, mishandling, etc.).
Policy:
Documentation of the alleged error and return of the original restoration are required before a complimentary remake will be considered.
7. Remake Request Time Frame
Policy:
All remake requests must be submitted within 30 days of the original in-office delivery. Requests submitted after this period will be processed as new cases and billed at the standard rate.
8. Remakes Due to Patient Preference
Policy:
Remakes requested due to patient dissatisfaction based on personal preference, esthetic subjectivity, or changes unrelated to laboratory error will be treated as new cases and invoiced accordingly.
9. External Design Responsibility
Policy:
If a case is designed by an external party not affiliated with or certified by Zenith Dental Arts, the laboratory assumes no responsibility for design-related complications. Any remake resulting from such issues will be billed at full price.
10. Technician Attendance and Clinical Supervision
If a Zenith Dental Arts technician is requested to be present for an in-office clinical procedure, the technician will operate exclusively under the supervision and direction of the prescribing doctor.
If a technician or the owner is needed for any AOX cases in the office to take necessary records, there will be a charge of $275 an hour.
If needed for a shade match, there will be a charge of $100 per patient.
If any cancellation is made 2 days before, there will be a cancellation fee of $150. Cancellations must be done via phone or a written message via email or WhatsApp.
If ANY records have been taken by Zenith Dental Arts, we reserve the right to decline a release of set records taken by Frank or any technician.
Policy:
Zenith Dental Arts assumes no responsibility for any clinical procedure performed during in-office appointments.
11. Laboratory Right to Decline Records
To maintain the highest standard of quality, Zenith Dental Arts reserves the right to decline any impression or digital scan that does not meet technical or clinical requirements necessary for accurate restoration fabrication.
Policy:
Fabrication may be postponed until adequate records are submitted. If the prescribing doctor chooses to proceed against laboratory recommendations, the laboratory will not assume responsibility for any resulting complications. Any associated remakes will be billed in full.
12. Prescription (RX) Requirements – Florida Department of Health Compliance
Policy:
In accordance with regulations established by the Florida Department of Health AND ADA, all RX forms submitted to Zenith Dental Arts must include:
- The prescribing doctor’s signature
- The prescribing doctor’s professional license number
ALL instructions must be sent with the RX via Scanner or Written on a separate RX form.
We DO NOT accept changes or instructions via Text message or Phone calls.
NO EXCEPTIONS to this policy.
Cases submitted without this legally required information may be delayed, suspended, or declined.
Acknowledgment
By submitting a case to Zenith Dental Arts, the prescribing doctor acknowledges, understands, and agrees to adhere to all policies stated herein. These policies exist to ensure clarity, consistency, and the highest level of restorative excellence.
